Casualties: Unknown drops players into the role of a disposable test subject, one of thousands sent by unseen captors to recover lost cargo on a foreign planet. Survival is not guaranteed, and the game leans into its unforgiving nature as a core part of the experience.

Players will explore five distinct layers of an alien cave system, each presenting diverse and hostile environments to navigate. The simulator genre focus suggests an emphasis on careful resource management and environmental awareness as players push deeper underground.
